We are partnered with a large managed care provider in search of a Behavioral Case Manager for their team. The Behavioral Case Manager/LCSW, LMFT, LMHC is a very important role, as it supports the psycho-social aspects and programs involved in managed care.
Responsibilities of the LCSW, LMFT, LMHC:
- Managing personal queue in appropriate systems
- Outreaching to members to assist them with identified complex case management needs
- Documenting all interaction with members/providers/other team members in company systems
- Calling providers to assist with appointments, prescriptions, other health related needs
- Assisting with SDOH needs including arranging transportation
- Checking and responding to email timely throughout the day
- Checking and responding to IM’s timely during day
Education/Experience of the LCSW, LMFT, LMHC:
Masters of Social Work or similar
LCSW, LCPC, LMFT, LMHC or equivalent
Managed care experience preferred
